For established clients and new clients who have scheduled an appointment or enrolled in a program.

Before You Begin
Please complete your forms at least 48 hours before your appointment whenever possible. This ensures we can use your visit time efficiently and focus on your priorities.
- Use a laptop or tablet if possible (easier for longer forms)
- Have your current medications and supplements nearby
- If you have recent labs, upload them in your portal (or email as instructed)
What do you need today?
- Log In to Your CharmHealth Portal → (PASTE CHARM LOGIN URL)
- Complete New Client Intake Forms → (PASTE CHARM INTAKE FORM URL or portal section)
- Upload Labs / Documents → (PASTE CHARM UPLOAD URL or portal instructions)
If you don’t have your portal link, check your appointment confirmation email or contact support below.
Common Forms (as needed)
This section is optional. If CharmHealth holds everything, you can keep this short.
You may be asked to complete one or more of the following based on your visit type.
List :
- New Client Intake — medical history, timeline, goals
- Health Toxicity Assessment — symptom and exposure score
- Stress Assessment — nervous system and stress patterns
- NET Wellness Check — mind-body screening and priorities
- Women’s Hormone Form — cycle/menopause patterns and symptoms

How the Client Forms Portal Works
3 steps:
- Sign in using the link provided
- Complete forms and save/submit
- Message or upload files if requested
Telehealth visits are typically via Zoom (with phone/FaceTime available when needed).
